Is there anything to check on the hydrant before the ground freezes?

Two things are worth confirming: that it drains fully after use, and that the ground around the base isn't staying saturated. A hydrant that can't drain will freeze in the standpipe. Fixing it after the ground hardens is much more disruptive.

Do frost-free taps need a slope to work?

They do. The barrel has to pitch slightly downhill toward the outside so it empties on its own once the handle closes. If a tap was installed dead level, or if settling has tipped it the wrong way, it holds water no matter how carefully it's shut off.

How do I know if a spigot leak is inside the wall?

The usual sign is that everything looks dry outside while a stain or a musty smell shows up on the inside of the same wall, and it worsens only while the tap is running. Old plaster hides it for a long time. Once it's suspected, keeping that tap shut is the safe move.

My outdoor tap was replaced but still freezes. Why?

Most often the replacement is the right type but the wrong length, so the valve seat ends up sitting in the cold section of the wall instead of inside the warm space. Wall thickness varies a lot in older housing. Measuring the assembly against the wall is what settles it.
